Had this bike never been assembled before?

If not, it is not unusual for stems, esp. Cinelli, to be slightly oversized. (The final polishing and finishing of stems is apparently a mostly hand operation and if you sight down the quill of many stems, you may be surprised at the undulations in the surface!) It is also not unusual for steerer tube interior to require gentle machine tool reaming.. The tolerances are extremely close and both must be dialed in by a pro mechanic with some experience. It is also common for the head set lock nut to not-be-concentric with the bore of the steerer tube.. Again gentle reaming of of this lock nut can accomplish a correct fit.

The Steerer tube is never faced or milled.. Maybe you mean the head tube? The head tube should have that done to ensure all the parts fit in the same plane (parallel to each other) but it theoretically should not affect the stem quill fit.

You mean that a SR lock nut threads on the steerer? Yes, that should preclude the chance that it is a metric steerer..
