I have a light tandem that makes this noise from the syncronising chainwheels and chain. It does it under high load but also when loaded at a standstill, which is how I could track it down. When loaded the frame twists and allows the chainwheels to move slightly out of line. The chain slips slightly sideways where it engages the alloy chainwheel from an angle under load and then centres on the chainwheel a bit further around and you hear the noise.
