My 1930s style Bianchi Class city bike came with one. It's supposed to be horizontal on top of the bars centerd over the stem. Holds an umbrella, a bagette, newspaper, etc. for the gentleman riding to work in Milano.
It could hold a vino bottle on its side... its not for a water bottle though.
