For these little touches on frames I've always seen it this way.

Frames in the 70s have no water bottle fittings, custom buillders start putting them on their bikes as a custom touch. Production sees it is popular, so production bikes start putting them on about 1980. Then custom builders put them on the seat and down tube, production bikes follow.

Same thing with the chain hangers. I am sure though, that with recessed brakes, it was dependent on who it was that made the recessed nuts. I got hold of some in 1984 that I would assume were made in the Orient, and fixed up several bikes with recessed brakes. Just had to be careful with those pivot bolt measurements. (Don't answer the phone in the middle of it)
