On the thread of custom shoes, the ones advertised as Rocket-7 (www.rocket7.com) come to mind. There are four or five different models to choose from, with prices that match. Remember, anything labeled, "Custom" ain't cheap. They even do custom footbeds for your existing shoes.

Always wanted this one but alas couldn't afford one so I scored big on E-bay, $300.00 including shipping. Purchased from a seller in Boston, selling for a friend, but the kicker is that the frame came to me with a Cycles D'Oro shop sticker on it!! The man's everywhere!!
Also, I used to work in an Orthotic and Prosthetic laboratory, before I joined the Army, and the people at Rocket-7 make there shoes the correct way. The order form is complete and precise.
