I agree with Mick that it is a late 1950s or 1960s Hetchins with the Experto Crede lug pattern (my fav!) .. Looks like the mid-term, non-cast lug version.. The filets in the head lugs do appear to be where the lugs were "broken" to change the angle, although that frame appears to be a middle size range where that step might normally not be necessary.
