In terms of function there is no practical difference whether the spacer is uppermost or otherwise. However, if the spacer is aluminum, and the washer steel, I assemble my bikes with the spacer uppermost. Then the center of gravity of the bike is lowest. I believe this is often referred to as the moroni method ;-)
