Dave, Like others have said it is 99% probality that it is the cotter pins. Check out Sheldon Brown's website about cottered cranks. We deal with cotter pins every day at work, mostly on old British three speed, sometimes its the angle of the cotter pin, cotter pins put in incorrectly(incorrect direction on both) but in very rare cases, the spindle actually twist. You can tell by using two rulers and putting them on the flats of the spindle. They Should be parallel. If it is twisted, only a matter of time before it breaks. Most of the time it is worn out cotter pins. Campy cottered bottom brackets are really hard to come by.
