One of the things I noticed when I regularly rode with Regina freewheels is that some inside cogs will "cup" toward the spokes, particularly the 22-tooth cog with holes in it. Is this part of the problem, perhaps? Look carefully at the spacing and if the gap between the two largest cogs is bigger than the others, then you have a freewheel problem, not a hub or spacer problem.
