Jerry, Your LBS can order you this one from QBP, part #CR0500. If you don't have a good LBS nearby, just get it from Harris.

Or this much cheaper ($25-$30) one from J&B, part #20226

(The J&B item is just the adaptor, the ISIS spindle and bolts shown in the illiustration are separate items.)
