Obvious suggestions, first. Check the roundedness of that piece and that it should not have too sharp edges. Rotate it as you push it on, as to find the best orientation with respect to the bent (and never perfectly round) handlebar tubing. Clean the handlebar thouroughly.

Should all these not be sufficient, get a better handlebar.
