I think that Brian has it pretty much nailed (or, I don't recognize the bike, but do see some cleverness), but I think he might be off on one point: I don't think that the braze-on (like a shifter braze-on) on the down tube just above the bb is for a demultiplicator. The demultiplicator arms would have just about centered on the bottom bracket, I think. Instead, I think it's a mount for a Sturmey-Archer-type pulley to carry the derailleur shift cable from the down-tube path to the chainstay path. An elegant solution to a problem that hasn't bothered most of us much - the sliding friction of cable against bb bracket or tunnel.

It looks like an old school homebuiltish touring bike made from Nervex P ro lugs (modified a bit) and useing a variation on the clamp type stem s ystem. Looks like a brazeon for demultiplictor(sp), has braze ons for ru bber chain slap thing, and so on. Not sure why the drilled steerer, exce pt for weight savings. It's all a bit ugly in my opinion. But maybe that 's the trud brown color talking.
