Amir, i've seen many axels like this...often a washer,outside the drop out...or spacer,inside the dropout, was present that had two tabs that faced the drop out to help locate the axel within the dropout as well as to keep the axel from spinning as you tightened the nuts....the flats were useful for hub overhaul at the vice so no threads got damaged...none flatted axels required an axel vice( an insert tool placed in the jaws) to protect the threads or simply 2 small blocks of pine wood...many of these were sadly separated from the bike when flats were repaired....look at any work stands parts tray and there is always an assortment of spacers and washers that didn't make it back to the bike but got substituted for by a careless repairman... SA hubs have a beautiful positioning washer complete machineing to add grip... good luck w/ your bike...charlie flaherty in baltimore maryland
