Below is a summary of the responses I got to my question about flats on axles:

1. 3-speed hubs had flats on axles to prevent any possibility of the the sun gear turning when torqued.

2. 3-speed hubs and other nutted hubs had flats on axles to enable single handed/tool opening and closing of the nuts. (This required a match between hub and dropout)

3. Flats make it easy to work on the hub.
