Even if one possesses the illustrious Campy puller, one should never attempt to extract a cork from the neck of such an old Port.

Instead, the neck is heated with hot tongs below the level of the venerable cork, and then the neck is entirely severed, cork and all inside, with a sharp stroke of a sturdy knife blade.
