Perhaps it became stripped or otherwise damaged? Or the owner wanted to use a derailleur that didn't quite work with the Campy hanger (e.g. Simplex or Huret without the special Campy D.O. adapter)?

Assuming you can replace it, little to no effect.

If you have one you can use for this. Campy made a special part to replace damaged hangers, but they're fairly uncommon. Or you could sacrifice the hanger from another dropout. But the easiest fix would be to use a bolt-on hanger.
