First. Do you have any cable left coming out of the head? If so, cut it down to just enough to get a grip on it with needle nose pliers. Push push push. The cable will be short enough to act as a rod and pop the head out. Works like a charm..usually.

You can drill from the back if you have no cable left. Use the correct size bit. Not a dremel bit. Too brittle. Secure the lever so it doesn't go spinnin' all over the place. Oh, and wear eye protection. drilling into frayed cable can be hazardous.
