My campy SR crankarm failure caused me to break my shoulder blade and have a bunch of stitches due to severe road rash. I also cracked my off-topic (but a good idea) helmet in two.

However, I'm pretty sure Laurent Fignon's problem was a broke Titanium spindle. I believe that TI Raleigh had some special Titanium crank arm spindles custom made for their team that were oversized and possibly nutted on the end instead of hollow and using bolts.
