I have no desire to go back to cottered cranks, but I have no experience of one failing. As long as they are installed correctly, and you are conscious of any looseness that might develop, there is no reason why one should fail. I have witnessed people riding where the crank flopped over tdc because the cotter pin was loose. 5 seconds tightening it, restores its function. Are we just looking for things to worry about?

One thing people don't think about; If a cotter goes bad you replace the cotter.  If a taper goes bad you replace the whole crank.
