The two 2 speed freewheels, one mounted on each side of the hub, were not used with a derailleur. You got off the bike, loosened the wingnuts, moved the chain with your fingers (or flipped the wheel around for the other two gears), and retightened the wingnuts and then got back on the bike. The cogs were one tooth apart because of the limitation of the length of the dropout and taking up the chain slack.

The Vittoria Margherita shifted a three or four speed freewheel. Same with the Osgear Super Champion.
